I, the undersigned, declare this art piece as my own original work and state that it is not a reproduction of another artist’s work. In submitting this art piece for consideration, I declare that it complies with the Richmond Public Library Code of Conduct for a Respectful Environment. I understand that it is my responsibility to remove the art piece at an agreed upon date, to be discussed with Library staff upon acceptance of my proposed display. I understand that the Library bears no responsibility for loss of or damages done to this art piece during its installment, removal or time of display.
